2012 Falcon Basketball Camp (Current 4th-8th Grade)----------------
June 18th through June 22nd at Poolesville High School from 9:00 AM to 3:30 PM.
Cost: $200.00 per Participant (sibling discount available---see below).
Camp Directors: PHS Varsity Basketball Coaches Tom Lang and Fred Swick.
To sign up now for the Falcon Basketball Camp, click here for email registration. Please include the player name, current grade and camp (Falcon, Junior, Mini-Falcon) they are attending. You will receive a confirmation that you have been added to the camp list. When you receive your email confirmation, please send your check (made out to the PBA) for $200.00 to the PBA at P.O. Box 332, Barnesville, MD 20838. Additional brothers or sisters pay only $180.00 each if they attend the Falcon or Junior Falcon Camps ($10.00 discount per session if the sibling attends the Mini-Falcon Camp). Make sure to also include all other email addresses (especially non-AOL) you would like PBA information sent to.
Participants may bring their own basketball with their name clearly written on it. Participants need to bring a brown bag lunch with drink to camp each day. Packing everything in a cooler is OK---just make sure your player's name is clearly written on whatever is brought in.
2012 Junior Falcon Basketball Camp (Current 2nd-3rd Grade)----------------
June 18th through June 22nd at Poolesville Baptist Church from 9:00 AM to 3:30 PM
Cost: $195.00 per Participant (sibling discount available---see below).

2012 Mini Falcon Basketball Camp (Current K, 1st and 2nd Graders)----------------
June 18th through June 22nd at Poolesville Baptist Church from 9:00 AM to 12:00 Noon (1st session) and 12:30 to 3:30 PM (2nd session).
Cost: AM session at $105.00. PM session at $90.00. Both sessions at $185.00 total. (sibling discount available---see below).

To sign up now the "Mini-Falcon" Basketball Camp, click here for email registration. Please include the player name, current grade and camp session they are attending. You will receive a confirmation. Send your check (made out to the PBA) for the proper amount to the PBA at P.O. Box 332, Barnesville, MD 20838. Additional brothers or sisters receive $10.00 discounts. Make sure to also include any other email address (especially non-AOL) you would like PBA information sent to.

Consent / Information Form
**BRING THIS FORM WITH YOU ON THE FIRST DAY OF CAMP!**
The participant and parent/guardian assume all risks associated with participation in the Falcon, Junior Falcon, and Mini-Falcon Basketball Camps and also the Falcon Soccer Camp. The Poolesville Basketball Association, also known as the PBA, including board members, administrators, coaches and other employees assumes no liability for injury or damages arising from participation in the camps. By signing this form, I accept all financial and medical responsibilities for my child and authorize the management of the Falcon, Junior Falcon and Mini-Falcon Basketball Camps and also the Falcon Soccer Camp to obtain emergency medical care for my child’s injuries or illness that might occur during the camp.
Due to the strenuous nature of some activities, the PBA encourages each participant to consult his or her physician concerning fitness in order to participate in the camps. The undersigned agrees (both personally and on behalf of the participant) not to sue, and releases, waives, and discharges the PBA, its coaches, volunteers, other participants, officials, and the PAA from any or all loss or damage.
The participant and parent/guardian also consent to PBA use of any photographs and/or videotapes that are made of the camp. Assuming the participant is a minor, the parent or guardian approves of his or her participation in the camp.
